What are your hours of operation?
Starting in August of 2025, we are scheduling in-office patients:
Mondays: 8am – 4pm
Wednesdays: 8am – 4pm
Special requests for other days or times can be made with our office staff, but may not be able to be accommodated at this time.

- Our office is located on the 2nd floor of the Landmark Oaks business building.
- After parking in the main lot, make your way to the lobby elevators.
- Head to the 2nd floor, exit the elevators and turn right down the hallway.
- Our office is at the very end of the hallway.
*Telehealth appointments are also available for those who cannot travel to our office.

How do I schedule an appointment?
To schedule an appointment you can:

2. Fill out our online form and our office will reach out to you by the end of the day.
What do I need to do before my appointment?
Please fill out your new patient intake forms before coming to your appointment.
What accommodations do you have for anxious patients?
One of the intake forms you will fill out before you arrive is our anxiety intake form. This gives you the chance to express worries and past experiences you would like us to be aware of. To the best of our ability, we will make your visit peaceful and anxiety free.
A few accommodations we make include:
- Attention to details, such as dimmed lighting to accommodate migraine patients.
- Weighted blankets available for appointments and procedures.
- A clean and comfortable waiting room with a short wait time.
- Meeting a doctor who cares about you as a person and a patient.
- A run down of all our pricing BEFORE you choose to get a treatment.

OROFACIAL PAIN
Who is a good fit to see an Orofacial Pain Specialist?
We treat jaw pain, nerve pain, headaches, and sleep apnea – all of which are hard to treat without a correct diagnoses. If you have felt frustrated, or like no provider can pinpoint your pain, we are here to help.
Orofacial Pain specialists’ training overlaps multiple fields of medicine, making them the ideal providers for chronic pain disorders:
- Dentistry: As Doctors of Medicine in Dentistry (DMDs), they have the knowledge to determine if pain is coming from a tooth, a nerve or somewhere deeper.
- Neurology: Orofacial Pain Specialists receive advanced training in the neuroanatomy of the face, especially with disorders of the cranial nerves.
- Sleep Medicine: They are given advance training in sleep dentistry and are uniquely suited to manage the side effects of sleep appliance therapy.
- Psychology: Orofacial Pain Specialists are trained to account for the impact that anxiety, stress, and depression have on chronic pain patients. We will never suggest you are imagining or exaggerating your pain. We will acknowledge the importance of your mental health and wellness in your recovery plan.
What conditions do you treat?
Orofacial Pain Conditions and Disorders Include:
- Temporomandibular muscle and joint (TMJ) disorders
- Issues with jaw movement
- Chronic headaches and migraines
- Nerve pain in the face, tongue, teeth, or gums
- Burning Mouth Syndrome
- Sleep disorders and sleep apnea
- Pains or sensations such as tinnitus, zapping, or tingling in the head and neck area

How are you different from a general dentist or ENT doctor?
While both of these providers may receive some training in treating TMJ disorders (TMD) and face pain, an Orofacial Pain specialist has received years of dedicated training exclusively in managing these conditions. Their knowledge overlaps with multiple fields of medicine, making them the ideal providers for chronic pain disorders.
What special training did Dr.Guthrie receive?
New regulations require all Orofacial Pain Specialists complete:
- A doctorate level degree in dentistry
- An additional 2-3 years in an Orofacial Pain residency program
- Yearly continuing education courses in Facial Pain
To become a board certified diplomat of the ABOP you must pass two additional examinations.
Dr.Guthrie has completed all of the above.
TREATMENTS
How soon can I receive treatment?
Our goal is to have you feel relief from your symptoms and receive a personalized treatment plan starting your first visit.
How do you treat TMJ Disorders?
Most TMJ Disorders can be treated with temporary life style adjustments, physical therapy, and if necessary, medications or occlusal appliances to aid in the healing process.
In some cases injection therapy can be helpful:
- Trigger point injections can help break-up “stuck” muscle fibers, allowing them to function better while reduce pain.
- Botox injections can be helpful to reduce the damage caused by years of overused jaw muscles.
- Platelet Rich Fibrin (PRF) injections can help repair and reline your joint surfaces after damage from arthritis.

How do I know what type of headache I have?
The best way to get a definitive diagnosis is to schedule an appointment with a provider who has had advanced training in managing headache disorders, such as an Orofacial Pain specialist or headache neurologist. With uncommon headache symptoms, a primary care provider may not have the training to diagnose your specific headache condition.

How often do I have to come in to treat my Sleep Apnea?
In the beginning, we recommend returning to the clinic every 2 weeks, to adjust the appliance and monitor for side effects. These appointments are included in the price of your appliance. Once we have your appliance extended to the proper setting, we only need to see you once per year to check that the appliance is still functioning properly.

What types of nerve pain do you treat?
We treat all nerve pain conditions in the mouth and face:
- Trigeminal neuralgia
- Pre-trigeminal neuralgia
- Post-herpetic neuralgia
- Occipital neuralgia
- Glossopharyngeal neuralgia
- Geniculate neuralgia (nervus intermedius neuralgia)
- Burning mouth syndrome
- Post traumatic trigeminal neuropathic pain
- Persistant idiopathic facial pain
- Persistant idiopathic dentoalveolar pain
- Red ear syndrome
- First bite syndrome
PRICING
Do you take insurance?
Yes, we do take insurance – although we are out of network with most major providers.
This is strategically done so your care is not delayed by pre-authorizations or your medications dictated by agencies unaware of your conditions. Our goal is to provide you excellent care, giving you relief on a quick timeline, with the cost communicated clearly upfront.
To accomplish this, financing at our practice looks like the following:
1. You will receive pricing for all appointments and treatments upfront, and you decide which ones to proceed with.
2. All payments are made at the end of your appointment, or financed via CareCredit.
3. We submit claims to your insurance on your behalf (Similar to a “Superbill”)
4. Your insurance will refund what they cover or the cost will be added to your out of pocket expenses.
How can I submit claims to my insurance company?
Our office does curtesy submissions which means it’s one less stressful thing for you to handle.
If you prefer to submit a claim on your own, we are happy to provide all the correct codes for you on a Superbill.
How much do your services cost?
A new patient appointment costs $500.
At your first appointment, you will receive a personalized treatment plan that includes the costs of everything listed.
You are in control to pick what to proceed with and will never have surprise fees.
