*Note that any treatment including medications or X-rays that are offered during this appointment are not included in the new patient appointment price.
New Sleep Patient Exam & Sleep Appliance
$2,400
For those who have been diagnosed with Sleep Apnea from a sleep study and want to treat it without a CPAP machine.
What’s included:
✓ Initial in person sleep consultation with Dr. Guthrie
✓ 40 minutes long
✓ Custom Mandibular Advancement Device
✓ Additional 3-5 adjustment appointments to keep the appliance fitting correctly
*If you are not ready to commit to a sleep appliance quite yet, you can book a sleep consolation for $200. That will go toward your final price if you move forward with treatment.
Emergency Appointments
$200
For those who have sudden jaw changes and are locked open or closed . We will do our best to see you asap and return your jaw to the correct position.
What’s happens:

✓ Brief exam to get locking diagnosis
✓ Noninvasive therapy to unlock your jaw
✓ Any Necessary Medication Prescriptions
*Note that many times buying a temporary biteguard after is recommended to support your jaw.

Do you take insurance?
We are out of network with insurance carriers and are a self-pay office.
This is strategically done so your care is not delayed by pre-authorizations or your medications dictated by agencies unaware of your conditions. Our goal is to provide you excellent care, giving you relief on a quick timeline, with the cost communicated clearly upfront.
To accomplish this, financing at our practice looks like the following:
1. You will receive pricing for all appointments and treatments upfront, and you decide which ones to proceed with.
2. All payments are made at the end of your appointment, or financed via CareCredit.
3. We can guide you through the process of submitting claims to your out-of-network insurance coverage (“Superbill”)
4. Your insurance will reimburse you for services that they cover.
How can I submit claims to my insurance company?
Our office can guide you through the process of insurance reimbursement submission and give you all the information you need via a Superbill.
How much do your services cost?
A new patient appointment costs $450.
At your first appointment, you will receive a personalized treatment plan with suggested treatments. The remaining costs vary from patient to patient depending on the diagnosis given during the new patient exam. You are in control to pick what to proceed with and will never have surprise fees.
